## About

These five sheets give fifth graders distinct ways to reflect on school learning: remember a learning moment, describe a challenge and a change, notice a contribution, explore an interest, and plan a next step. Each prompt asks for a four-sentence paragraph with a specific detail or connection. Optional sentence starters support the writing, and twelve ruled lines provide room for each response.

One privacy line appears on the first sheet. Students can leave out names, use a supplied example or pass; public sharing is not required. The last sheet includes a self-check with a separate space to add a missing detail or connection, or record that the response already answers its prompt.

The separate two-page teacher guide includes a worked model, five response exemplars with acceptable alternatives and boundaries, misconception feedback, response supports, and scoped W.5.4/W.5.10 practice mapping. Teachers can read aloud, rehearse, scribe or support AAC responses. The activity can be used at a checkpoint during the year or near its end, with time to reflect and revise.
